Notes on performing a medical diagnosis
The appropriate time to undergo tests
a. Hematology test
- The appropriate time to undergo the test
– In general, doctors usually recommend having a blood test in the morning. This is because at this time, the body’s indicators are more stable, waste substances in the blood have been eliminated, and the organs are functioning more effectively.
– In addition, taking the test in the morning helps patients avoid fasting for too long. Most blood tests require fasting for about 8–12 hours. If done in the afternoon, patients would have to avoid eating or drinking throughout both the morning and lunchtime. Moreover, since the body is more active during the day, it is easier to feel fatigued.
- Notes
– If you are taking medication, consult your doctor for specific advice on when to stop certain medicines, including some used for blood pressure or diabetes, etc.
– Avoid stimulants such as tea, coffee, alcohol, and smoking before the test to prevent any impact on the results and subsequent treatment.
b. Urine/Stool test
For tests such as urinalysis, before collecting a sample, patients should carefully clean the external genital area with clean water. Do not use alkaline or acidic cleansing solutions for douching, as this may affect the accuracy of test results.
- Correct method of collecting a urine sample:
– Ensure hands and the external genital area are thoroughly cleaned. Collect the urine sample without letting your hand touch the inside of the specimen container (the container should be labeled with the patient’s name and date of birth). Discard the first stream of urine, collect the midstream portion into the sterile container (about two-thirds full), then close the lid tightly and hand it to the laboratory staff.
– For stool tests, patients should be instructed to collect samples from areas that contain mucus, blood, or liquid stool.
Breast cancer screening
Early Breast Cancer Screening Process at Thien Nhan Hospital
Clinical Examination
Customers will be examined directly by specialist doctors. The doctor will take a personal and family medical history, evaluate any symptoms (if present), and conduct a clinical breast exam to detect lumps or abnormal lymph nodes. After the examination and risk assessment, the doctor may provide counseling and recommend additional necessary tests.
Blood Test
- CA 15-3 tumor marker test to aid in the early detection of breast carcinoma.
- Genetic testing for hereditary breast cancer to support screening, prognosis, and prevention of inherited breast cancer.
Imaging Diagnosis
- Breast ultrasound: Detect abnormalities such as breast tumors, breast abscesses, etc.
- 3D mammography: Screen for breast cancer, especially useful in detecting microcalcifications and structural distortions that may not be found by clinical examination or ultrasound.
- Breast MRI: Ordered by doctors in cases such as:
– Checking breast implants and parenchymal lesions if present
– Lesions not forming masses on ultrasound and mammography
– Identifying the nature of breast tumors, determining whether they are multifocal or unifocal
– Pre-surgical or neoadjuvant chemotherapy staging assessment
FNA and Core Biopsy
If suspicious breast lesions are found on imaging, doctors may recommend FNA (fine needle aspiration) or core needle biopsy for cytology and histopathology testing to confirm diagnosis and advise on treatment.
Notes for Breast Cancer Screening with 3D Mammography
- Do not apply powder, ointment, or cream to the chest area before the test as it may distort the imaging results.
- Do not undergo the test if pregnant, suspected pregnant, or breastfeeding, or if the breast shows signs of pain due to inflammation, abscess, etc.
- Patients with kyphosis, wheelchair users, or women with breast implants will be given appropriate technical guidance.
Gastrointestinal Endoscopy Procedure (Stomach and Colon)
Notes before undergoing Endoscopy
- For endoscopy services, patients will first undergo a clinical examination by specialists before being recommended for the procedure to ensure safety and effectiveness after endoscopy.
- To achieve the most accurate results, patients should:
– Fast and limit water intake for at least 6 hours prior to the procedure
– For colonoscopy: take bowel cleansing medication 2 to 4 hours before the procedure
– On the evening before the examination: eat easily digestible food; avoid hard-to-digest food, red meat, and fruits with seeds such as dragon fruit, watermelon, etc.
– On the morning of the examination: do not eat breakfast, do not drink milk, coffee, etc.
Step-by-step Procedure for Gastroscopy and Colonoscopy
Clinical examination, medical history review, and necessary tests as prescribed by the doctor (if any).
The endoscopy technician provides medication and instructions:
- For gastroscopy: Simethicone anti-foaming medication is given to reduce bubbles in the esophagus, stomach, and duodenum, improving visibility and accuracy in detecting small lesions and preventing missed diagnoses.
- For colonoscopy: Bowel-cleansing medication is administered to ensure a smooth procedure, prevent missed lesions, and maximize safety during interventions.
Receive endoscopy clothing from the technician and enter the endoscopy room. The endoscopist will explain and provide necessary counseling about the procedure. Then, the anesthesiologist will administer anesthesia to ensure the patient feels no pain or discomfort during endoscopy.
The doctor inserts the endoscope equipped with a camera and light into the patient’s digestive tract. The images of the gastrointestinal tract are displayed on a monitor, allowing the doctor to observe and detect any abnormalities.
During the procedure, if necessary, the doctor may perform biopsies, polyp removal, hemostasis, or foreign body extraction.
After endoscopy, patients will rest in the recovery room and receive a complimentary light meal.
Once the endoscopy results are available, patients will have a direct consultation with a specialist doctor.
